This career opportunity as a full-time Patient Intake Coordinator is responsible for providing customer service to potential patients/families, professional referral sources, and colleagues to effectively and efficiently coordinate referrals and respond to inquiries about home health services.

What you will be doing:
- Receive and process referrals and inquiries, attend new prospect meetings, ensure proper staffing of all client classes, and work closely with administrators and other office personnel to ensure client satisfaction.
- Promote our homecare services to physicians, hospitals, skilled nursing facility managers, discharge planners, and case managers to build and maintain client relationships.
- Prepare a business plan and maintain targets, prospect lists, and leads. Spend at least 50% of your time in the field with office team support, discussing home care needs with new clients and prospects.
- Prioritize accounts and gather and organize account-related information to provide input on key customer opportunities and service line extensions.
- Process applications and background checks for potential employees and schedule personnel for assignments.
- Develop positive relationships with all clients and referral sources, and maintain absolute confidentiality of all information pertaining to employees, clients, and client's family members.
- Maintain documentation according to policies and procedures and exercise independent judgment within Agency guidelines.
What we are looking for:
- A background in customer service, concierge experience, sales, business development
- High school diploma and two years of experience in an office setting, preferably in health or home care
- Excellent written and verbal communication skills with a customer-centric outlook and some sales skills preferred
- Proficiency with Microsoft Office (Word, Excel, and Outlook), scheduling systems, and other healthcare industry-related software
- Ability to remain flexible and resilient, maintain a sense of humor, and present a calm, professional image.
- Ability to plan, organize, prioritize, delegate, and accurately follow through in work activities with time constraints and interruptions to meet deadlines and work independently with minimal direction and/or supervision.
